SERVICES
- WHO IS ELIGIBLE?
To be eligible for our services potential
clients will have to meet all of the following:
Must be an older person or a person
with a disabilty.
Must live in the community.
Must have an ongoing functional disability
and have difficulty in performing every
day tasks (e.g. gardening, cleaning etc.).
Must be on a low income such as an aged
or disability pension.
Prior
to receiving services clients will need
to be assessed for eligibility using the
HACC Needs Identification Tool (HNI). These
assessments are completed by VTF staff and
may be done by phone or in the person's
home.
